A physical medicine and rehabilitation doctor can enhance an assisted living facility in several ways. Here are the key areas where their specialized knowledge improves care for residents.

For residents

Nine ways a physiatrist improves resident care.

Rehabilitation and Mobility

Create individualized rehabilitation plans to help residents regain or maintain strength, balance, and functional abilities.

Pain Management

Using various methods, including medication, injections, and physical therapy techniques to improve a resident’s comfort and well-being.

Fall Prevention

Assess fall risks and implement preventive measures, including exercise programs, medication adjustments, and environmental modifications.

Diagnosis and Treatment

Diagnose a wide range of medical conditions affecting seniors, and treat or collaborate with other specialists to ensure comprehensive care.

Care Plan Development

Work collaboratively with the facility’s medical team to develop and oversee individualized care plans that address each resident’s specific goals and needs.

Collaboration

Physiatrists act as team leaders, working closely with healthcare professionals to ensure coordinated care for residents.

Education and Training

Educational programs for staff on pain management, fall prevention and rehabilitation techniques, to improve the overall quality of care the facility provides.

Competitive Advantage

A physiatrist on staff is a significant selling point for ALFs, attracting residents who require more specialized care and rehabilitation services.

Improve Resident Satisfaction

Effective pain management and increased mobility, with a focus on functional medicine, can lead to high satisfaction among residents.

About us

A rehabilitation group built by physicians, for patients.

Watch

About Pacific Rehab Consultants.

Focus on patients,
we handle the rest.

PRC was founded by Dr. Michael Decker, DO, to improve and optimize care delivery through communicative and educational collaboration within each facility. We remain the only 100% physician owned and operated rehabilitation group.

We provide physiatry care in skilled nursing facilities across California and the Pacific Northwest, and we take on the administrative work, facility communication and billing with insurance companies so a clinician's time stays clinical.

With you every
step of the way.

Since founding PRC, Dr. Decker has aimed to raise the bar and improve overall outcomes for the patients of skilled nursing facilities through communication and collaboration. In 2023 Dr. Tyler Doornink established PRC's Pacific Northwest chapter, serving Eastern Washington and Northern Idaho.

Values

The values that shape everything we do.

Our ten core values, in our own words.

01

Service to Others

How we show up for our patients, our communities, and our families.

02

Take Initiative

We actively look for and find problems to solve, then we solve them.

03

Act with Integrity

We do what we say we're going to do with regards to the patients and facilities. It not only helps the patients to build confidence in us, but we also build confidence in ourselves.

04

Be Present

We are where our feet are. We show up, we do what we say we are going to do, and we take care of any issues in front of us.

05

Always Be Learning

We strive to continue to grow, not only from an educational standpoint, but in all the things we don't know.

06

Be Humble

Humility is one of the cornerstones to being a physician. When we're with patients, we don't always have to know the right answers, but we have to express to them that we want what's best for them and we will find out the answers.

07

Details Matter

Some of the smallest details mean the most not only to the patients and facilities, but also their outcomes.

08

Take Ownership

We go above and beyond and we accept not only patient responsibilities, but also the outcomes that the patients and facilities have.

09

Be Grateful

We really appreciate where we are in our lives and the interactions we get to have with our patients.

10

Give Generously

This is what we do when we're at the facilities, it's what we do in our lives, it is how we interact with our world. Because we have so much, we do our best to give back.

Physiatry

What is physiatry, and why post-acute facilities want it.

Physiatry is physical medicine and rehabilitation, the medical specialty focused on restoring function after illness, injury or surgery.

The specialty

Physicians who treat function, not just a diagnosis.

A physiatrist is an MD or DO who completes residency training in physical medicine and rehabilitation. They diagnose and treat conditions of the muscles, bones, nerves and brain that affect how a person moves and lives, and they lead the rehabilitation plan rather than a single procedure.

Function first

The goal is mobility, independence and a return to the patient's optimal state, measured against goals set with therapy.

Team leaders

Physiatrists coordinate therapy, nursing, primary care and specialists around one plan.

Non-surgical

Medication, injections, bracing and therapy prescriptions, with surgery as a referral, not a default.

Documentation that holds up

Clear notes that support the need for therapy, survey criteria and audits.

In post-acute care

Where a physiatrist fits in a skilled nursing facility.

Post-acute rehabilitation

From admission through a safe discharge, with rounding twice a week and length of stay decided on real progress. Read more

Pain management and procedures

Peripheral joint and trigger point injections that reduce pain so therapy can do its work. Read more

Long-term residents

Evaluating functional decline and the benefit of therapy so residents progress back to baseline. See the SNF program

Training

What the training looks like.

Physiatrists complete medical school, an internship year, and a residency in physical medicine and rehabilitation. Some, like PRC's Dr. Kyaw KZ Lin, add a fellowship in pain medicine.

PRC's physicians trained at programs including the University of California, Irvine, NYU Langone Health and Pacific Northwest University of Health Sciences, and the group's founder is board certified in the specialty.

Bring physiatry into your building.

Speak with a dedicated PRC team member on a quick virtual meeting.

Physiatry

Post-acute rehabilitation, from admission to a safe discharge.

How a PRC physiatrist manages a patient's stay in a skilled nursing facility.

The stay

What happens between admission and discharge.

1. Evaluate

A physiatrist evaluates each patient and sets rehabilitation goals with the therapy team, addressing anything that could limit progress.

2. Round twice a week

Follow-up on therapy progress, acute medical issues, pain, DVT prevention and pressure ulcer prevention, with nursing and family kept informed.

3. Discharge safely

Length of stay is extended or reduced on real progress, and discharge recommendations are made for a safe return to the community.

Why it matters

Consistent oversight changes outcomes.

It is our experience that patients do significantly better with consistent oversight utilizing goals set by therapy, with a focus on motivational strategies.

Clear, organized documentation supports the need for therapy and holds up to RAC audits and state and federal surveys.

Early family, caregiver and social worker communication makes for the smoothest transition of care.

Direct communication with orthopedic, vascular and spinal surgeons promotes healing and carryover of recommendations.

Long-term residents are evaluated for functional decline and the potential benefit of therapy.

Add a physiatrist to your facility.

Speak with a dedicated PRC team member on a quick virtual meeting.

Physiatry

Pain management and procedures that let therapy work.

Pain that goes untreated stalls rehabilitation. PRC physiatrists manage it inside the facility.

In the building

What our physicians perform on site.

Peripheral joint injections

Performed in the facility to help reduce pain and enable increased benefit from therapy.

Trigger point injections

Performed to help reduce pain and enable increased benefit from therapy.

Medication management

Pain regimens reviewed as part of clinical management, alongside the primary care physician.

Specialist coordination

Evaluating for a higher level of care and communicating directly with surgeons when needed.

Part of the whole plan

Pain care is part of the whole clinical picture.

Clinical management

Recommendations for pain management sit alongside rehabilitation, acute medical issues, DVT prevention and pressure ulcer prevention and management.

Seen twice a week

Physicians round on each patient twice per week to follow up on therapy and rehabilitation progress.

With primary care

We work closely with primary care physicians to make sure each patient's clinical needs are met.

With the surgeons

Direct communication with orthopedic, vascular and spinal surgeons, and evaluation for a higher level of care when needed.

More than one tool

Medication, injections and therapy, used together.

Medication

Physiatrists prescribe medications and order and interpret diagnostics as part of each patient's care plan.

Injections

Peripheral joint and trigger point injections to help reduce pain and enable increased benefit from therapy.

Therapy

Physical therapy techniques to improve comfort and function, followed up on progress at every visit.

Fall prevention

Assessing fall risk and recommending exercise programs, medication adjustments and environmental modifications.

For assisted living too

Pain management is one of nine ways a physiatrist improves resident care.

Using various methods, including medication, injections, and physical therapy techniques to improve a resident's comfort and well-being.
